What to bring to your business appointment…

When we consult with you or prepare a tax return for you, we like to be as thorough as possible, while being efficient with your time. Therefore, we have put together a checklist of things to bring to your appointment.

When you come for your appointment, please bring the following (if applicable):

  • Completed Business Entity Information Form
  • Prior year tax return(s) (for new clients)
  • State Organization Documents: Please bring copies of your articles of incorporation, partnership or organization and other documents related to the filing of your organization with the state, including any DBA’s (Doing Business As) or trademark
  • S-Election: If your business is an S-corporation, please provide a copy of the Form 2553 that was filed with the IRS and a copy of the IRS acceptance letter
  • Employer Identification Number: If you have filed for an employer identification number, please provide a copy of the Form SS-4 and a copy of the IRS letter acknowledging your number
  • Income and Expenses: Please provide income and expense information for your business including a full back-up of your QuickBooks data with user name and password if you utilize this program. Otherwise, please provide this information in whatever format you currently have it.
  • Business Bank Statements and Business Credit Card Statements: Please provide twelve months of statements, along with bank reconciliation documentation
  • Payroll and Subcontractors: Please provide copies of all employment and subcontractor related tax forms including W-3/W-2’s, 1096/1099’s, state unemployment quarterly filings, and federal forms 940 and 941. If not prepared yet, please provide names, addresses, social security numbers and amounts paid
  • Inventory: Documentation related to any inventory owned by the company at the beginning and end of the year
  • Business Assets: Information related to all tangible and intangible assets owned by the business, including depreciation schedules, a copy of your tangible tax return, documents showing purchases and sales for current year
  • Sales Tax Returns: Copies of any sales tax returns for the year
  • Completed Business Questionnaire
